Confidentiality Agreement

I understand that I am about to be granted access to confidential information regarding patients for whom I may not be the primary referring practitioner. I hereby agree that any information accessed on this platform will be used solely for medical purposes to benefit the wellbeing of the patient and aid in their treatment. I acknowledge my obligation to abide by the Privacy Act set out in the Privacy Commissioner’s National Privacy principles.
